Are in-person services available?
Yes, in-person services such as vaccinations, immunizations, blood-pressure checks, and diabetes management are available at Well Revolution Care Locations. These services are offered at the best cash prices and may be covered by your insurance.
For everything else, your care team will coordinate care with local, in-network specialists or facilities known to offer care at non-exorbitant prices.
Can I get controlled medications with my membership?
Yes, where medically necessary and following a continuity of care approach. There is a 30-day stand-down period to build a trusted relationship with your provider, ensuring they fully understand your history, health, and goals.
Where is membership available?
Memberships are currently open to everyone in New York state who are 18 years or older. Limited membership are available on a first come, first served basis.
If the state in which in you live is not currently listed, you may be eligible for membership, or we will notify you when membership slots are available in your state.
